The New England Journal of Medicine shares some of the most amazing imagery from the often weird and wonderful field of medicine, but sometimes the content can be simultaneously disgusting and yet mesmerizing.

On Tuesday, the widely respected journal shared a truly incredible case: a 36-year-old man with aggressive end-stage heart failure had a coughing fit so extreme that he coughed up a six-inch wide blood clot.

While that, in and of itself, is something astounding, the medical staff treating the unnamed man were astonished to find that, given his unique medical circumstances, the man had actually expectorated a perfectly intact cast of his right bronchial tree, one of the tubular networks that brings air to and from the lungs.

Georg Wieselthaler, a transplant and pulmonary surgeon at the University of California at San Francisco, was in charge of the man’s care, and hooked the patient up to a pump to assist blood flow throughout his body, while also providing a course of anticoagulants to help keep the man alive.

“You have high turbulence inside the pumps, and that can cause clots to form inside,” Wieselthalers told The Atlantic. “So with all these patients, you have to give them anticoagulants to make the blood thinner and prevent clots from forming.”

However, anticoagulants can cause problems if a breach occurs in the blood-vessel network,  which happened in this extraordinary case; blood broke out of the patient’s pulmonary network into his lower right lung.

“We were astonished,” Wieselthaler said. “It’s a curiosity you can’t imagine—I mean, this is very, very, very rare.”

It turns out that the 36-year-old patient’s condition produced elevated levels of fibrinogen, a protein that can act as a glue to assist the clotting process, which allowed the rubbery ‘blood sculpture’ to escape the man’s trachea intact.

“Because it was so large, he was able to generate enough force from an entire right side of his thorax to push this up and out,” Gavitt Woodard, a clinical fellow said.

Sadly, despite the best efforts of the medical team, the man died a week after coughing up the bronchial tube cast.

Bronchial tree clots (called casts) are actually relativelycommon but only in extremely rare cases do they emerge in such pristine condition.

The Fortnite week 4 challenges are live, and Express Online has got all the solutions you need.

The week 4 challenges are made up of seven tasks that range from dealing damage with a sniper rifle, to searching through supply drops.

One of the more difficult challenges tasks players with searching between a Vehicle Tower, Rock Sculpture, and a Circle of Hedges.

Fortunately, fans have already discovered the location of the Battle Star, which is found south-west of Moisty Mire.

The Battle Star appears in the H9 section of the map, in a small pool of water near the wooden hut.

It’s then just a case of collecting the Battle Star and performing a dance, like YouTuber ElChinoLOV3.

The Fortnite ice cream truck challenge is also live, and will reward players with five Battle Stars.

Fortnite fans need to find five different ice cream trucks spread across the giant Battle Royale map.

Fortunately, Fortnite Intel has put together a map showing the locations you need to visit in order to solve the challenge. You can check it out in the gallery above.

While hiking in the peak district of central Austria, a person might expect to gaze upon stunning mountaintop vistas of natural beauty. But one rambler stumbled across an object that certainly isn’t in the tourist guidebook – a giant wooden penis.

The large phallic sculpture was found on a rocky outcrop of the 1,800-meter (5,900ft) high Ötscher mountain. Measuring about 1 meter in height and etched with great detail, the mountaintop penis was discovered by blogger Marika Roth on the Lackenhof trail.

The nature writer posted images of the artwork to social media last Thursday, declaring that the popular Ötscher hiking trail had a new attraction. “I think the ötscher has a new mascot,” Roth wrote on Facebook.

It’s unclear who brought the graphic carving up the mountain. But it’s providing Facebook commenters with the opportunity to discuss the male anatomy and question the rationale behind its placement.

“The Ötscher is a sleeping giant… so someone has opened his pants,” an internet commenter stated. Others have debated whether a penis is better than a crucifix, which also sits on the mountain.

“Better a friendly penis that demonstrates fertility and strength than the fearsome execution of ancient times,” a second person wrote under the images.

Another chimed in: “Penis instead of summit cross… shows just what values some people have.” Whatever people’s view of the piece, Roth said it presented a “fun change” to her normal walk.

This dental care clinic took a novel approach to luring in customers – or rather, terrifying them into a visit – using a sculpture made from human teeth and the stuff of nightmares. Warning: you will most likely want to unsee this.

Emil Aghajanyan, General Director and Orthodontist at the Kind Dentist’s Clinic in St. Petersburg, Russia said they wanted something to remind customers about the importance of brushing your teeth.

“It’s also a reminder of how disgusting a person looks when they don’t tend to their dental cavities, don’t take care of their teeth and have bad breath,” Aghajanyan added.

Shock therapy indeed, the “kind dentists” believe that the morbid sculpture will haunt its viewers as a reminder to take care of their teeth no matter what.

“His main task is to frighten, to make people think. And I am sure that many people who have seen it and heard the main idea will never forget it,” Aghajanyan said, explaining why the monstrosity was installed in the clinic’s lobby.

Apart from being a pioneer in the use of art for dental shock therapy, Aghajanyan is also a blogger – his website “The kind dentist’s notes” has been going strong since 1997.

An Austrian teacher has been fired from his job after letting students create an oversized penis sculpture during his art class.

The unnamed teacher was employed at a high school in the small Austrian town of Waidhofen an der Thaya, near the border with the Czech Republic, and had gained a reputation for making inappropriate sexual remarks in front of his 12-year-old students, according to The Local.

Though the children reportedly found making the erect penis to be a bit of fun, word travelled fast in the small town of just over 5,500, and parents quickly complained to the school’s director, Roland Senk, once they found out just what their kids were up to.

Senk confirmed that the teacher was present when the children were constructing the model penis, adding that it could not be dismissed as a “harmless joke.”

The 49-year-old teacher appeared before the Lower Austrian School Board last week and was subsequently suspended and his temporary contract will not be renewed.

A life-size gold sculpture depicting rapper Kanye West as a crucified Jesus Christ, called “False Idol,” has surfaced in Hollywood.

The Oscar-like statue appeared on Hollywood Boulevard on Wednesday and is a representation of the way in which society idolizes celebrities, until they crucify them.

“The piece is as critical of us as consumers as it is of Kanye himself,” the street artist behind the creation, ironically named “Plastic Jesus,” told the Hollywood Reporter.

The Sapporo Snow Festival is now underway in Japan’s northern most prefecture Hokkaido. One of this year’s main sculptures is Final Fantasy VII themed, featuring Cloud and Sephiroth.

Impressive, no?

Each year, the Japanese military moves all this snow to make these huge sculptures.

Why is Cloud’s head so small? According to Famitsu, this was due to the snow’s weight. It was necessary to make his lower back larger so the sculpture won’t collapse.

At night, the sculpture is brought to life with projection mapping.
