LUNG cancer cases were missed in a Portsmouth hospital, after patients’ chest x-rays weren’t checked by properly trained radiologists, according to hospital watchdog, the Care Quality Commission .![]()
Three patients suffered “serious consequences”, after signs of lung cancer went unnoticed in the Queen Alexandra Hospital, Portsmouth, said the CQC.
There was a backlog of 23,000 chest x-rays from the previous 12 months at the hospital, it said. X-rays hadn’t been formally reviewed by radiologists, or appropriately-trained physicians.
Junior doctors were given responsibility for reviewing chest and abdomen x-rays, when they weren’t fully trained, and didn’t feel confident enough to do so, they told the watchdog.
The CQC has now ordered a review into all radiology reporting across the NHS in England. Portsmouth Hospitals NHS Trust has also been ordered to provide the CQC with weekly updates on the size of x-ray backlogs.
The hospital trust has “unreservedly” apologised to families of the three lung cancer patients.
CQC’s Chief Inspector of Hospitals, Professor Ted Baker, said: “When a patient is referred for an x-ray or scan, it is important that the resulting images are examined and reported on by properly trained clinical staff who know what they are looking for – this is a specialist skill.
“Some junior doctors told us that they had been given responsibility for reviewing chest and abdomen x-rays without appropriate training although they felt that they were not competent or confident to do so.
“We then learned of some cases where signs of lung cancer were missed, with serious consequences for the patients involved. This is clearly unacceptable.”
Portsmouth Hospitals NHS Trust was ordered to take urgent action to address the backlog of 23,000 x-rays, and to identify any patients that may have suffered harm.
An inspection of the hospital took place in May, where the trust was ordered to take immediate action to identify possible harm to patients.
CQC inspectors returned to the hospital in July, for a focused inspection of the outpatients and diagnostic imaging department at Queen Alexandra Hospital.
They found the backlog of chest x-rays, and were told of three serious incidents when patients with lung cancer had suffered “serious harm” because their x-rays hadn’t been properly assessed.
On the back of the findings, the CQC is now reviewing radiology reporting across the entire NHS in England. All NHS acute and community NHS trusts in the country were asked to give details of any x-ray backlogs, as well as turnaround times and staffing.
Portsmouth Hospitals NHS Trust said it had already cleared half of its x-ray backlog.
It’s Chief Executive, Mark Cubbon, said: “We have issued an unreserved apology to the families of the three patients who experienced harm because of the delays to their care. It is of deep regret to all of us, that we did not deliver the high standards of care everyone should expect from our hospital.
“We take the CQC’s concerns extremely seriously and fully accept the findings of the inspection report highlighting delays in reporting chest x-rays.
“We have worked hard to ensure that we are providing a timely service, and have carried out a thorough review of the scans and x-rays reported so far, to date nearly 50% of the backlog has been cleared and we are in touch with any patients as necessary.”
The Queen Alexandra Hospital has 1,200 beds, and has 6,300 full-time members of staff.

The Apple Watch has a heart rate sensor that has reportedly been saving lives, but it doesn’t return the kind of data a doctor needs to really assess what’s wrong with a patient.
Doctors looking to diagnose heart issues require what’s called an electrocardiogram test, or EKG for short — not readings from an optical heart rate sensor like those found on most wearables.
Now AliveCor, a private medical health company headquartered in Mountain View, California, is releasing a new Apple Watch band that has a built-in EKG sensor. According to CEO Vic Gundotra, it’s the first-ever medical accessory for a wearable that’s been approved by the Food and Drug Administration.
“We live in a world where consumers want to know every minute what’s going on [in their bodies] and for the first time they can have a medical, clinical-grade, FDA-cleared tool,” Gundotra said.
AliveCorAliveCor is calling the device KardiaBand. It costs $ 200 and is on sale from Amazon and AliveCor’s site, and you don’t need a recommendation for a doctor to use it — it’s meant for consumers, and anyone who might want to keep a closer watch on their heart or those concerned with heart issues like sinus heart rhythms or atrial fibrillation.
“The average consumer doesn’t know what a normal sinus rhythm looks like or what atrial fibrillation looks like. Yet the FDA has cleared our individual algorithms,” Gundotra said. “The consumer can have confidence that this is FDA-cleared. And frankly, we have the clinical studies to prove it.”
The KardiaBand slides into any Apple Watch and uses its own small watch battery, which Gundotra says should last two years. To take a reading, you simply place your fingers on the small metal contacts embedded in the band for 30 seconds. That will take an EKG reading, which is shown in real-time on the Apple Watch, and you can email the reading to your doctor.
AliveCorThe KardiaBand launching on Wednesday is actually the second version of the KardiaBand, Gundotra said. The first version launched shortly after its announcement in early 2016, but it was only approved in Europe.
The version on sale in the United States is packaged with one big improvement called SmartRhythm. That part of the AliveCor Apple Watch app uses the wearable’s built-in heart rate sensor to take readings every five seconds.
The feature uses machine learning models running on the watch itself, according to the company.
“We were able to run a deep neural network on the Apple Watch and keep it to 14 hours of battery life on the new Series 3 Watch,” Gundotra said. “Apple deserves kudos for building such a platform that allows us — I mean, I think we are the most advanced app ever built for the Apple Watch. “
If SmartRhythm detects that your heart rate is unusual — for example, it’s unusually high and you’re not on a run — it will tell you to take an EKG test to get a more accurate reading.
These software features aren’t free, though. They’re part of a premium subscription to AliveCor that costs $ 99 per year or $ 9.99 per month. Anyone using the KardiaBand also needs to subscribe to the premium service, which also provides unlimited storage for EKG readings and a mailed monthly report that users can bring to their doctors.
The fact that AliveCor can operate in the medical world with doctors but at the same time produce medical and health gadgets for consumers is the startup’s key advantage, Gundotra says.
“The members of our team are people who build consumer products and so we know what it takes to run a product that a 55 year old can use. And it’s not easy by the way,” Gundotra said. “If you look at medical products, a defibrillator, the pacemaker that my father have, they don’t show anything to the consumer. It’s all hidden to the consumer. The DNA of AliveCor is very different.”

Adam “Armada” Lindgren, considered one of the “five gods” of competitive Super Smash Bros. Melee, has dropped out of this weekend’s DreamHack Austin due to a controller that isn’t malfunctioning properly. Yes, you read that correctly.
The obvious question here is why is a professional player—with the resources of an organization behind him—isn’t prepared with multiple back-ups in case of a failure like this? The answer isn’t completely satisfactory, but it is fascinating.
In keeping with Melee’s legacy of pushing an old game at every corner, some Melee players opt to use controllers that are technically malfunctioning, in order to execute otherwise impossible moves on a standard controller. Since Melee is a 16-year-old game for the Nintendo GameCube, there’s no possibility for patching out certain bugs or exploits, so controllers have been one method for pros to consistently execute certain moves.
Here is how tech data enthusiast who goes by the pseudonym Kadano explained it on Twitter:
There’s a bug in Melee that only technically malfunctioning controllers can avoid. The malfunctioning is rare, volatile & causes other issues
Due to this malfunctioning being a necessary criterion as long as we don’t employ Magus’ smash turn fix code, only about 1-3% of GCCs are viable for players like @ArmadaUGS and @MVG_Mew2King, since that’s how few have said malfunctioning (PODE) to a sufficient degree.
The amount and type of PODE can change any time, so at the moment, the competitively best GCCs are inherently unreliable.
The only way around this is either employing Magus’ smash turn fix code or using controllers with digital buttons to dash / analyzer chips.
In layman’s terms, certain inputs that players like Lindgren use are only possible through the use of a controller that is technically malfunctioning in a specific way.
For example, a dashback—or back dash depending on your jargon—occurs when a player inputs a dash backwards from a standing animation. It’s a very useful move in competitive Smash, but executing that input consistently can be difficult if your controller has excessive snapback—when the analog stick vibrates slightly after moving from an outer position to a neutral position in the center. Players will run controllers through tests, ranking them on their ability to consistently execute dashbacks, in order to find a controller that will suit their needs.
As Kadano goes on to say, it’s difficult to find a controller malfunctioning in the specifically necessary way—Lindgren would have had to purchase roughly 50 controllers to find one that would fit this criteria.
Despite this, there were several competitors on-site who would have had the equipment as well. There’s no shortage of talent in Austin this weekend, and even one should be bound to have a controller that fits Lindgren’s parameters, right? Apparently, this was an issue Lindgren has been aware of for at least a week:
Still, Lindgren says he is “sad/mad” about having to drop out of singles in DreamHack, and will still be around to field-test controllers for use in his upcoming tournaments.
Though the mechanics of the reason why pros use these controllers is fascinating (if you dig the technical aspects, I highly recommend Melee It On Me’s guide to controller quality), it seems strange that there isn’t a more readily available option for Lindgren to get a controller (or several) that’s up to snuff. Though there are several sites that sell professional player-centric controller mods, they seemingly didn’t work for Lindgren, and he was forced to drop out.

VARNER, Ark. (AP) — A federal judge temporarily blocked Arkansas from executing a second inmate Monday night after attorneys questioned whether the first execution had gone properly.
Jack Jones received a lethal injection earlier Monday evening that lasted 14 minutes. The state was set to execute Marcel Williams about an hour later after the U.S. Supreme Court rejected Williams’ last appeals, but a federal judge temporarily halted the proceedings due to an emergency filing from Williams’ attorneys.
U.S. District Judge Kristine Baker halted Williams’ execution until she issues another order. His death warrant expires at midnight.
In the emergency filing, Williams’ attorneys wrote that officials spent 45 minutes trying to place an IV line in Jones’ neck before placing it elsewhere. It argued that Williams, who weighs 400 pounds, could face a “torturous” death because of his weight.
Intravenous lines are placed before witnesses are allowed access to the death chamber.
Arkansas’ attorney general says the attorneys’ account of the Jones execution was “inaccurate.”
An Associated Press reporter who witnessed the execution said Jones moved his lips briefly after the midazolam was administered, and officials put a tongue depressor in his mouth intermittently for the first few minutes. His chest stopped moving two minutes after they checked for consciousness, and he was pronounced dead at 7:20 p.m.
Jones, who’d argued that his health conditions could lead to a painful death, gave a lengthy last statement. His final words were: “I’m sorry.”
“I hope over time you can learn who I really am and I am not a monster,” he said in the roughly 2-minute statement.
Williams was scheduled to be executed at 8:15 p.m. Monday after the U.S. Supreme Court rejected his final appeals, but Baker’s order placed that on hold.
Jones was sent to death row for the 1995 rape and killing of Mary Phillips. He was also convicted of attempting to kill Phillips’ 11-year-old daughter and was convicted in another rape and killing in Florida.
Jones said earlier this month that he was ready for execution. He used a wheelchair and he’d had a leg amputated in prison because of diabetes.
The state conducted its first execution last week after a nearly 12-year hiatus. Initially, Gov. Asa Hutchinson scheduled four double executions over an 11-day period in April. The eight executions would have been the most by a state in such a compressed period since the U.S. Supreme Court reinstated the death penalty in 1976. The state said the executions needed to be carried out before its supply of midazolam, one lethal injection drug, expires on April 30.
The first three executions were canceled because of court decisions, then inmate Ledell Lee was executed last week.
Williams’ “morbid obesity makes it likely that either the IV line cannot be placed or that it will be placed in error, thus causing substantial damage (like a collapsed lung),” his attorneys wrote in a court filing asking justices to block the execution.
Both men were served last meals on Monday afternoon, Arkansas Department of Correction spokesman Solomon Graves said. Jones had fried chicken, potato logs with tartar sauce, beef jerky bites, three candy bars, a chocolate milkshake and fruit punch. Williams had fried chicken, banana pudding, nachos, two sodas and potato logs with ketchup, Graves said.
Before Lee’s execution Thursday, Arkansas hadn’t put an inmate to death since 2005. In several of the 31 states where executions are legal, drug shortages have often forced delays as manufacturers prohibit their use in executions. Arkansas believes that secrecy it grants to suppliers can solve that problem, though it still has difficulty obtaining the drugs. Courts have also forced rewrites of Arkansas’ lethal injection protocols, causing further delays. Jones and Williams committed their crimes more than two decades ago.
In recent pleadings before state and federal courts, the inmates said the three drugs Arkansas uses to execute prisoners — midazolam, vecuronium bromide and potassium chloride — could be ineffective because of their poor health.
Jones, 52, lost a leg to diabetes and was on insulin. Williams, 46, weighs 400 pounds, is diabetic and has concerns that the execution team might not be able to find a suitable vein to support an intravenous line.
The poor health of both men, their lawyers claimed, could make it difficult for them to respond during a consciousness check following a megadose of midazolam. The state shouldn’t risk giving them drugs to stop their lungs and hearts if they aren’t unconscious, they have told courts.
The last state to put more than one inmate to death on the same day was Texas, which executed two killers in August 2000. Oklahoma planned a double execution in 2014 but scrapped plans for the second one after the execution of Clayton Lockett went awry.
Arkansas executed four men in an eight-day period in 1960. The only quicker pace included quadruple executions in 1926 and 1930.
Williams was sent to death row for the 1994 rape and killing of 22-year-old Stacy Errickson, whom he kidnapped from a gas station in central Arkansas.
Authorities said Williams abducted and raped two other women in the days before he was arrested in Errickson’s death. Williams admitted responsibility to the state Parole Board last month.
“I wish I could take it back, but I can’t,” Williams told the board.
Jones was given the death penalty for the 1995 rape and killing of Mary Phillips. He strangled her with the cord to a coffee pot.
In a letter earlier this month, Jones said he was ready to be killed by the state. The letter, which his attorney read aloud at his clemency hearing, went on to say: “I shall not ask to be forgiven, for I haven’t the right.”
Including Jones, eight people have been executed in the United States this year, four in Texas, two in Arkansas and one each in Missouri and Virginia. Last year, 20 people were executed, down from 98 in 1999 and the lowest number since 14 in 1991, according to the Death Penalty Information Center.
