Treating patients effectively involves a combination of training and experience. That’s one of the reasons that people have been excited about the prospects of using AI in medicine: it’s possible to train algorithms using the experience of thousands of doctors, giving them more information than any single human could accumulate.

This week has provided some indications that software may be on the verge of living up to that promise, as two papers describe excellent preliminary results with using AI for both diagnosis and treatment decisions. The papers involve very different problems and approaches, which suggests that the range of situations where AI could prove useful is very broad.

Choosing treatments

One of the two studies focuses on sepsis, which occurs when the immune system mounts an excessive response to an infection. Sepsis is apparently the third leading cause of death worldwide, and it remains a problem even when the patient is already hospitalized. There are guidelines available for treating sepsis patients, but the numbers suggest there’s still considerable room for improvement. So a small UK-US team decided to see if software could help provide some of that improvement.

Osteopetrosis can cause a number of serious, potentially life-threatening problems throughout the body.

According to the US’s National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS), it is triggered by an imbalance in the formation and breakdown of bone.

This causes the bone to become overly dense, but also weak and at risk of breaking easily.

Unlike the similar-sounding osteoporosis – where bones become brittle and fragile from loss of tissue – people are born with osteopetrosis.

Bones that are too dense can interfere with important tissues and structures.

For example, compression of nerves in the skull can lead to vision loss, hearing loss, or paralysis of facial muscles.

The NIAMS warn that it can also trigger crowding of the bone marrow, which may lead to low levels of cells needed to fight infection, carry oxygen to the body’s cells, or control bleeding.

In some cases this case can prove deadly.

There are two main types of osteopetrosis – malignant infantile and adult. 

The former is apparent shortly after birth and can reduce life expectancy.

In contrast, the adult form may not be diagnosed for decades.

According to the NIAMS, it is caused by defects in one or more genes involved in the formation, development, and function of cells called osteoclasts.

These cells break down bone tissue during a standard process called bone remodelling, where old bone is removed and new bone is created to replace it.

The most common symptoms of osteopetrosis are bone fractures, low blood cell levels, impaired vision and hearing, and dental problems related to infection.

However, their severity varies from person to person.

The condition can usually be posted on x-rays, where bones appear unusually dense or chalky white.

Sarcopenia is a condition where muscle mass and strength is lost as you get older.

This can cause significant reduction in a person’s strength, which can also affect gait and balance, causing difficulty climbing stairs and lifting objects.

The condition is estimated to affect 4.6 per cent of men and 7.9 per cent of women with an average age of 67 years, according to research published in the journal Age and Ageing.

Muscle mass starts to decline from the age of 30 years, and it is estimated people will lose three to eight per cent of muscle mass per decade.

It has long been thought to be an inevitable part of the ageing process.

However, according to the International Osteoporosis Foundation, scientists have recently been researching whether it is possible to slow loss of muscle mass and strength.

While sarcopenia is most commonly seen in inactive people, it also affects those who are physically active throughout their life.

At the moment experts believe age-related factors that cause sarcopenia include hormone levels changing, protein requirements alter, motor neurone die and we tend to become more sedentary.

Additionally, poor nutrition, such as a lack of vitamin D, is thought to play a part.

The condition is associated with serious health issues, including frailty, disability, morbidity and mortality.

These are five main symptoms of the condition.

Muscle size

People may begin to notice a decrease in muscle size.

Unlike some other symptoms, it can be physical indicator of the condition.

Weakness

Feeling weak is another sign.

This can increase people’s risk of falling.

Endurance

Losing endurance – or stamina – is another indicator.

Physical endurance is important for being able to do everyday activities.

Balance

Experiencing poor balance can be a sign of sarcopenia.

This can lead to a person avoiding physical activities, which in turn can speed up muscle loss.

Stairs

Issues climbing stairs can also be caused by the condition. 

In the elderly, having sarcopenia can raise risk of falling.

Research by the University of Southampton has linked muscle loss with increased fractures and falls.

The Lions have announced full-back Hogg will return home to Scotland for further treatment on a “facial bone fracture”.

Hogg collided with Conor Murray’s elbow as the Ireland scrum-half sought to let the Glasgow star launch a counter-attack in the first half of the Crusaders clash.

“It is hugely disappointing for Stuart to have to return home early and for us to lose one of our touring party,” said Lions head coach Warren Gatland.

“Stuart has been a key member of our squad on and off the field and it is disappointing to see injury cut short his time with us in New Zealand.

“We all wish him the best with his recovery and we look forward to seeing him back in action in the future.”

No replacement has been called up at this stage, and defence coach Andy Farrell yesterday suggested the tourists already have enough options to offset Hogg’s absence.

Farrell said: “At this point in time, we’ve got a big squad and a lot of people in place to cover different positions.

“We picked a bigger squad this time round than we did for the last tour, simply because of the schedule – not just the games but the travel time and the amount of time we’re going to have on the training field together.”

Hogg would have been one of the main backline talents boss Gatland could have expected to deliver the X-factor.

Ireland’s versatile backline operator Jared Payne will wear the 15 shirt against the Highlanders this morning, while Leigh Halfpenny ought to be favourite to start at full-back for the Test series.

Hogg offered a good luck message to the Lions squad as he prepared to fly home.

“I’m gutted to be leaving the Tour at this stage,” said Hogg.

“It is an honour to have been involved in a second British and Irish Lions tour and I have hugely enjoyed my time with the squad and I’m very excited about the potential of this group.

“I’m disappointed to be leaving in this way but wish all the squad the best in the coming weeks and will be cheering them on from home.”
