Emergency medical services are the go-to for a system that frequently fails to address social determinants of health, which Barilla says are often entirely overlooked. “These are essentials of life,” she explains. “It’s food, security, housing, safety and socialization.” Other social determinants include factors such as exposure to violence — be it intimate partner abuse, elder abuse or community violence — as well as access to education, employment and childcare.
“If you don’t have health, it’s hard to function in anything else,” says Christopher Lee, clinical solutions marketing manager for Family Health Centers of San Diego. “What good is it if you have health care but don’t have proper housing?”
Or any housing, for that matter. Lee points to the particular challenge of treating the homeless, who not only lack stable housing but also move from place to place every few days. “How do you have any continuity of care with that?” he asks.

The answer to this question, and others, could depend on starting “upstream” from the actual point of care. This upstream concept stems from a belief that, by proactively helping broader communities and providing social services that address each community’s specific needs, health care systems can help reach the people who are most likely to wind up in the ER or forgo care altogether.
“We’re finding that housing, food, security and even social connections are really key for emergency prevention,” Barilla says. “We can help create communities that will support optimal health, which is where we need to go as a nation.”
PSJH operates a number of programs designed to implement this upstream approach, with a large focus on the problem affecting most major cities: homelessness. “We work on coordinated entry programs, rapid rehousing if someone loses their housing, and medical respite care for the homeless who have been discharged from a hospital but are not OK to be on the street,” Barilla explains.
Anchored Home Initiative, an upstream program in Alaska, is a collaboration between PSJH, the United Way, Alaska’s Housing Authority and other local community partners. It is dedicated to ending homelessness in the Anchorage area. “This program is excellent because it offers a continuum of care,” Barilla says, “not only housing, but supportive services homeless people might need such as Medicaid, mental health counseling and case management.”
To show how an upstream approach to health care can be at once so effective and so simple, Barilla offers an analogy: Picture a hospital located on a curved and busy roadway, the site of many accidents due to a lack of safety guardrails. The hospital brainstorms solutions to deal with the growing number of patients: add more ER beds, hire more specialty neurosurgeons to treat trauma patients, arrange for a helicopter to airlift victims to nearby trauma centers. All fine ideas, but even better and far less costly? Build guardrails and put up more cautionary traffic signs.
“That’s upstream care,” says Barilla.
It’s true that there’s a shift underway in how health care is practiced in the U.S. — especially with regard to Medicaid expansion across various states — but Barilla believes that treatment is still too focused on wealth and class: “We’ve been rationing care through economics, and socioeconomic disparity is very significant.” Her solution is to invite in more public and private partners.
“The reason programs like Anchored Home Initiative are going to work is because everyone is on board,” Barilla points out. “That program has more than 75 partners and organizations involved — everything from law offices to restaurants to police departments.”
Lee seconds this approach, agreeing that one organization, one city or one state can’t make a lasting impact: “There has to be better aligning of resources in order to address these issues early on.”

President Donald Trump criticized China in a tweet on Thursday following multiple reports that Chinese companies have been secretly selling oil to North Korea.
The sales would be in violation of UN sanctions against the Kim Jong Un regime and could threaten to derail Trump’s diplomatic attempts to denuclearize Pyongyang.
“Caught RED HANDED — very disappointed that China is allowing oil to go into North Korea,” Trump tweeted. “There will never be a friendly solution to the North Korea problem if this continues to happen!”
The Chosun Ilbo, a prominent South Korean news agency, reported Wednesday that US reconnaissance satellites in the region have spotted Chinese ships trading oil with North Korean vessels dozens of times since October. The report is based on satellite images and several sources within the South Korean government.
Ship-to-ship trading with North Korea was barred by a UN security council resolution passed in September, which strengthened sanctions on Pyongyang following a nuclear test launch earlier that month.
Trump has been both praiseworthy and critical of China’s cooperation in handling North Korea. In a series of tweets in July, he railed against China for doing “nothing” for the US on North Korea.

Following a meeting with Chinese President Xi Jinping in November, Trump appeared more confident in China’s willingness to cooperate, calling Xi a “very special man” and lauding his work ethic.
“We must act fast, and hopefully China will act faster and more effectively on this problem than anyone,” Trump said at the time, referring to the nuclear threat posed by North Korea. “China can fix this problem easily and quickly. I know one thing about your president: If he works on it hard, it will happen.”

The usually consistent, reliable gelding ran the worst race of his career last start in the Railway Stakes in Perth, failing to be a rival to the line in the Group 1 race. His official losing margin was a whopping 49.75 lengths!
This kingsize flop was less than a month ago but such is the respect for Supply And Demand he is likely to be favourite at $ 3.50 with TAB Fixed Odds to quickly rebound from the Ascot disaster.
Adrian Bott, who co-trains Supply And Demand with Gai Waterhouse, conceded it is hard to find excuses for the poor Perth run.
“It is difficult to explain what was a very disappointing performance in Perth,’’ Bott said.
“He probably felt the firm track and maybe he didn’t travel that well, either.
“But since he has been back home, he is bright and healthy, he has settled in nicely and we really can’t fault him in anyway.’’
Bott said a strong track gallop at Randwick on Tuesday morning convinced him the gelding was ready to race at Warwick Farm.

Supply And Demand worked over 1200m on the dirt, recording 1m 17.1s including a brilliant final 400m in 22.31s. He also galloped strongly at Randwick.
“His work on Tuesday morning gave us a lot of confidence going into Saturday,’’ Bott said.
“I think he should be assessed on his first two runs this campaign and not what happened last start. His first two runs back were super, they were very, very strong efforts.’’
Supply And Demand reeled off four successive wins to end his four-year-old season including the Caloundra Cup and Grafton Cup before resuming with a fast-finishing second to stablemate Cabeza De Vaca at Randwick.
He then contested the Group 3 Chatham Stakes during the Melbourne Cup Carnival at Flemington and again hit the line strongly for a close second to Helova Street prior to his Railway Stakes failure.
“The slight query about (Saturday’s) race is he is coming back to 1400m at this point of his preparation,’’ Bott said.
“But hopefully we have done enough to freshen him up. He has his share of weight but he is a class horse.’’
Supply And Demand is the only Warwick Farm runner for the Waterhouse-Bott stable but they do have three starters at Moonee Valley tomorrow including promising stayer Imperial Aviator in the Bertocchi Christmas Masterpiece Handicap (2500m).
Imperial Aviator, who began his race career in England, was impressive scoring his first win down under at Rosehill last start over 2400m.
“We have sent Imperial Aviator down to Melbourne to give him a preparation there ahead of the spring carnival next year,’’ Bott said.
“Because we have the Flemington stables, it is a huge asset to us and we can send a horse down there, giving them experience and exposure to the travel and a different environment.
“This is only Imperial Aviator’s second Australian preparation and his two runs back this time in have been very good with a second at 2000m then to win second-up at 2400m, that is not easy to do.
“It takes a particular class of horse to do what he has done and we are hoping he can develop into a Cups-style of horse next spring.’’
Bott is also starting in-form filly Aeecee Tong De in the Gold Carat (1200m) and Multitude lines up in the Summer Membership Handicap (2040m).
“Aeecee Tong De is a nice sharp filly who has really strengthened up this time in work,’’ Bott said.
“She has always had speed and precocity but didn’t have the physical strength until now. She is developing into a promising filly.
“We worked her at Moonee Valley last Tuesday and Stephen Baster said her gallop was very good. Imperial Aviator had a look around the track, too.’’
Bott said Multitude has been improved by one run back from a short let-up and is expecting a competitive effort.
Meanwhile, the stable’s glamour three-year-old filly and brilliant Victoria Oaks winner Pinot will return to training next week as she prepares for an autumn campaign aimed at the Group 1 ATC Australian Oaks at Royal Randwick during The Championships in April.
“Pinot’s had a good break but it is a bit of a tight turn around off a big Oaks campaign,’’ Bott admitted.
“It is not an easy task but if we can bring her back and she can resume where she left off in the spring, it will be very exciting.’’

Under the leadership of Pablo Escobar, the Medellin cartel moved immense amounts of drugs and brought in stratospheric profits.
By one measure, they were raking in $ 420 million a week by the mid-1980s, and by the end of the decade the cartel reportedly supplied 80% of the world’s cocaine.
Such prodigious trafficking made Escobar and his organization the target of the Colombian government and its close partner, the US.
The two countries launched a brutal campaign against him, and Escobar responded in kind, leaving Colombia littered with bodies until his own death on December 2, 1993.
Escobar’s legend has grown in the years since. Similarly, the drug trade has continued apace, despite Escobar’s demise and the withering away of the Medellin cartel.
“What happened with the Medellin cartel after we took them down? The Cali cartel got stronger, right? And we then took them down. Norte de Valle cartel takes over; we take them down,” Javier Peña, a US Drug Enforcement Administration agent who worked on both the Escobar and the Cali cartel cases, said on The Cipher Brief podcast.
The Cali cartel had long been a Medellin rival, but it reached the height of its power after Escobar was killed, eventually controlling 80% of the world’s cocaine trade and moving hundreds of tons of it to the US. The Norte de Valle cartel emerged from the remnants of the Cali cartel in the late 1990s.

Both Medellin and Cali would spawn numerous offshoot criminal groups that would continue to operate in Colombia.
“We’re taking down cartels and another cartel is born, and what we tell people is as long there’s a demand for this nasty type [of] drugs that are out there, there’s going to be people that are going to take a chance in sending their dope to make a quick dollar,” Peña said.
A similar dynamic has been seen in Mexico over the last decade, as government-led efforts (backed by the US) to cripple major cartels by taking out senior leadership — the so-called Kingpin Strategy — instead spurred both intra- and inter-cartel fighting.
The most recent example of that dynamic is the turmoil within and around the Sinaloa cartel. The arrest of cartel chief Joaquin “El Chapo” Guzman in February 2014 — followed by his escape in July 2015 and rearrest in January 2016 — appears to have kicked off fighting among his sons and lieutenants for control of the cartel, as well as drawn attacks from erstwhile allies and ascendant rivals.

Efforts to arrest the drug trade through police action need to be coupled with civil-society efforts to keep drugs from infecting communities, Peña said.
“You still need that enforcement effort,” he said. “However, I think we need to get better in our society as far as not getting involved with this type of stuff.”
Asked if he thought a project like Nancy Reagan’s “Just Say No” campaign in the 1980s was needed, Peña agreed.
“We need to see more involvement from everybody,” he added. “There’s going to be dope. People look at the heroin we’re facing right now in the United States … at one point it was the methamphetamine; now we’re hearing more about the heroin epidemic, the opioids, the prescription drugs.”
“The solution — I wish I had the answer,” Peña continued. “However, we just need to get better with society, the schools, the families, churches, religion.”

Peña is not the only one who was closely involved with bringing down Escobar and now touts a more holistic approach.
Cesar Gaviria, elected president of Colombia in 1989 — weeks after Liberal Party candidate Luis Galan was assassinated (reportedly at Escobar’s direction) — called for a less security-centric approach to the war on drugs.
Gaviria was writing in reference to the bloody anti-drug campaign Philippine President Rodrigo Duterte has waged since taking office in June 2016.
“Illegal drugs are a matter of national security, but the war against them cannot be won by armed forces and law enforcement agencies alone,” Gaviria wrote. “Locking up nonviolent offenders and drug users almost always backfires, instead strengthening organized crime.”
“Taking a hard line against criminals is always popular for politicians. I was also seduced into taking a tough stance on drugs during my time as president,” he added. “We started making positive impacts only when we changed tack, designating drugs as a social problem and not a military one.”
Duterte, for his part, responded by calling Gaviria an “idiot” a few days later.

On Wednesday, the ECJ said that it had “dismissed in its entirety the actions brought by Slovakia and Hungary,” which aimed to have the quota system annulled.
The ruling follows an EU decision made in 2015 to rehouse some 160,000 refugees from Greece and Italy over a period of two years, only around 27,700 of whom have been settled so far. Hungary and Poland have refused to take part, while Slovakia has only taken in a small number from Greece. The low level of relocations, the ECJ noted, was partly due to “the lack of cooperation on the part of certain member states.”
“It is time to be united and show full solidarity. The door remains, it is still open, and we should convince all member states to fulfill their commitments,” EU Migration Commissioner Dimitris Avramopoulos said.
“But we should be clear that member states have to show solidarity now because it is now that some member states need help.
If the member states do not change their approach in the coming weeks, we should then consider [taking] the last step in the infringement procedure: to refer Poland, Hungary, and the Czech Republic to the European Court of Justice.”
Commenting on the ruling, Germany Foreign Minister Sigmar Gabriel said it was a positive step that fully adhered to European values and law. The decision is also likely to be welcomed by other countries such as Greece and Sweden, where the sheer volume of new arrivals threatens to overwhelm the system.

However, Gabriel’s Hungarian counterpart, Peter Szijjarto, took the opposite view, declaring the ECJ’s ruling unacceptable.
“The Hungarian government considers today’s decision by the European court to be appalling and irresponsible,” Szijjarto said at a news conference. “This decision jeopardizes the security and future of all of Europe.”
“Politics has raped European law and values,” he added.
Human rights groups have criticized Hungary and Poland for their reluctance to take in any refugees. The government of Hungarian Prime Minister Viktor Orban has taken the stance that Hungary has the right to refuse entry to the primarily Muslim asylum seekers and refugees, claiming they threaten the country’s cultural identity. In September 2015, a razor-wire fence was constructed along the border with Serbia and Croatia, blocking migrants trying to reach the EU via the Balkans.
“The lesson of the verdict is that helping people fleeing war and terror is truly a common responsibility for Europe,” the Hungarian Helsinki Committee, which provides free legal assistance to asylum seekers, said in a statement quoted by AP. “Hungary needs to respect the decision of the EU court.”
Polish Prime Minister Beata Szydlo also rejected the ECJ’s ruling, saying that Poland will hold firm to its position on asylum seekers and refugees.
“I was convinced that such a decision would be made [by the court], but this absolutely does not change the stance of the Polish government with respect to migration policy,” Szydlo told reporters at a business conference.
Poland initially accepted a quota of several thousand refugees, but quickly changed its mind after the 2016 terrorist attacks in Brussels, citing security concerns.
But Slovakian Prime Minister Robert Fico said he accepted the court’s decision.
“We fully respect the verdict of the European Court of Justice,” Fico told reporters, but added that his government still views the system of quotas imposed by Brussels negatively.
“We will continue to work on having solidarity expressed in different ways other than forcing migrants [on Slovakia] from other countries that don’t want to be here anyway.”
Over the past few years Europe has been faced with millions of refugees and asylum seekers escaping war zones and hardship from across Africa and the Middle East. While the flow of Syrian refugees has slowed somewhat since a deal was signed with Turkey, the question remains of what to do with the ones that have already arrived.
