  • Drugmaker Pharmacyclics had tripled the cost of a lifesaving cancer blood treatment when it introduced a new version of the pill.
  • The new price tag faced criticism from cancer doctors who were concerned about how this would impact how they dosed patients with the drug. 
  • Last week, Pharmacyclics reversed its decision to take the initial version of the drug off the market, giving patients and doctors a lower-cost option. 

The maker of a lifesaving blood cancer treatment planned to triple the cost of the drug — but it’s backing down after pressure from doctors. 

The drug, Imbruvica, treats serious forms of blood cancer such as leukemia and lymphoma. Developed by Pharmacyclics in conjunction with Johnson & Johnson’s Janssen arm, it has increased survival rates especially in high-risk patients and most patients treated with the drug show favorable outcomes.

Celgene is buying Juno Therapeutics. 

The biotech giant is acquiring the cancer drugmaker in a $ 9 billion deal, Celgene said on Monday. Celgene, which already owned 9.7% of Juno’s shares, will pay $ 87 per share in cash.

Juno is known for its experimental, highly personalized cancer treatments called CAR T-cell therapy (CAR is short for chimeric antigen receptor).

“The acquisition of Juno builds on our shared vision to discover and develop transformative medicines for patients with incurable blood cancers,” Celgene CEO Mark Alles said in a news release Monday. Celgene, which has been under pressure from investors to make some changes after a rocky 2017, has now made two acquisitions this month, after picking up Impact Biosciences in a $ 7 billion deal on January 7. 

2017 was a big year for these CAR-T treatments: the Food and Drug Administration approved two treatments, one to treat treat pediatric acute lymphoblastic leukemia in people up to age 25 and another to treat aggressive B-cell non-Hodgkin lymphoma. And in August, Gilead Sciences nabbed CAR-T drugmaker Kite Pharma in a $ 12 billion deal. 

Juno’s farthest-along treatment, JCAR017, is expected to be approved in the US to treat another type of blood cancer  called diffuse large B-cell lymphoma in 2019. Celgene said the treatment has the potential to hit $ 3 billion in global sales. 

How CAR-T cell therapy works

These treatments aren’t your run-of-the-mill pill — or even a biologic drug, like insulin — that can be mass produced. Since the therapy is made from a person’s own immune system, the process can take about three weeks.

  • To start, a doctor removes some white blood cells, the part of our body’s immune system responsible for combatting infections and foreign substances, from a patient. In a healthy body, the immune system can recognize abnormal, cancerous cells, but for people with cancer, it doesn’t recognize that the cells are spreading.
  • Then the cells are taken to a manufacturing facility at which point the cells are reengineered to recognize cancer cells and wipe them out.
  • Those reprogrammed cells are sent back and administered to the patient.

While the treatments don’t work in all patients, it can have dramatic results in those who do respond. For example, in a trial of 63 patients treated with Kymriah — the first cell therapy approved — 83% were in remission after three months, and 64% were still in remission after a year.

‘A big new field of medicine’

With two CAR-Ts already approved and more in the works over the next few years including Juno’s, the field of cell therapies is starting to emerge.   

“We’re at the very beginning of what’s going to be a big new field of medicine,” David Epstein, who helped license Kymriah from the University of Pennsylvania while at Novartis, told Business Insider in August after Kymriah was approved.

Epstein left Novartis in 2016 as CEO of its pharmaceuticals divisions. He’s now the executive chairman of Rubius Therapeutics, a biotech firm that’s also working with cell therapy to develop treatments like the CAR-Ts that don’t have to be as personalized. The hope is that one day doctors will be able to prescribe a cell therapy and use it that same day instead of waiting weeks to get it back.

Epstein said he envisioned cell therapies having much shorter life cycles than traditional drugs. Instead of getting a better, updated therapy for a disease every decade or so, we might begin to see second-generation cell therapies in a few years.

One challenge these therapies still face is how toxic they can be. CAR-T’s side effects can be deadly. In May 2017, Kite disclosed that one person had died in a clinical trial for its late-stage CAR-T therapy from cerebral edema, a condition in which excessive fluid causes the brain to swell. Juno said five people in its clinical trials had died, all from cerebral edema.

Eventually, cell therapies could go beyond blood cancers, and include solid tumors and maybe even autoimmune diseases like Type 1 diabetes, Epstein said.

Biotech giant Celgene is in talks to buy cancer drugmaker Juno Therapeutics, The Wall Street Journal reports. 

Celgene has been under pressure from investors to make some changes after a rocky 2017. Already in 2018, the company acquired Impact Biosciences in a $ 7 billion deal, a move that didn’t entirely excite the biotech community. 

Juno declined to comment on the report. 

Juno is developing a highly personalized cancer treatment called CAR T-cell therapy (CAR is short for chimeric antigen receptor). 2017 was a big year for these treatments, with the Food and Drug Administration approving two, and Gilead Sciences buying CAR-T drugmaker Kite Pharma for $ 12 billion. 

A deal between Juno and Celgene wouldn’t be entirely out of the blue — the two companies have been partnered since 2015 in a $ 1 billion deal that gave Celgene certain rights to commercialize Juno’s treatments. 

Juno was up 40% Tuesday evening on the report.

Drug manufacturers have been feeling the heat over the price increases they make on prescription drugs. 

Sanofi, which makes drugs to treat diabetes, along with vaccines and heart medications, has been criticized in the past for the price of its insulin Lantus. 

To counter that, the French drug giant has decided to make some of the strictest drug pricing commitments in the industry. 

Among Sanofi’s new pricing promises,

  • The company committed not to increase the list price of existing drugs in the US by more than the rate of health inflation, as measured by the Centers for Medicare and Medicaid Services’ National Health Expenditure. In 2015, the NHE rate is expected to be 5.4%, and it’s expected to increase on average 5.6% through 2025 years.
  • Sanofi also plans to launch new drugs with a lot of rationale around how it set the price.
  • The company also decided to release its average list and net price increases going forward. It’s something other companies have been doing as well. 

The approach is a bit of a departure from other players in the pharma industry. In September, Allergan committed to only single-digit drug price increases (a line it closely hugged for some drugs). Novo Nordisk, another diabetes drugmaker, also committed to capping increases to single digits.

Cybele Bjorklund, Sanofi’s head of global policy, said the company decided to use the NHE as a benchmark because it’s independent and has some level of predictability. “Single digit is somewhat arbitrary,” she said. 

And if that inflation rate were to go down? “We’re willing to live with wherever it goes,” she said. 

As far as the rationale goes for new launch prices, Sanofi will have to show why its drug is better than what’s currently available. Bjorklund gave the example of Dupixent, a drug to treat atopic dermatitis. When the drug launched in March, Sanofi and its partner Regeneron made headlines for setting a price that was lower than expected. 

As part of the new price policy, Sanofi began disclosing its average list price increases. On the whole, Sanofi’s list price increased 4% in 2016, but after factoring in discounts and rebates paid to pharma middlemen, the net price decreased by 2.1%.

In part, that’s because of its long-acting insulin Lantus, which hasn’t increased in list price since 2014, but has been facing pressures from pharma middlemen.

April 27 (Reuters) – AbbVie Inc’s quarterly profit and revenue beat analysts’ estimates, helped by demand for its top-selling treatment, Humira.

Humira, used to treat forms of arthritis and Crohn’s disease among other conditions, generated sales of $ 4.12 billion, just ahead of analysts’ estimates, accounting for nearly 63 percent of the company’s total revenue.

Investors have been jittery over looming biosimilar competition for the world’s top-selling drug, which generated more than $ 16 billion in sales last year.

AbbVie, which is trying to block the U.S. launch of a Humira biosimilar developed by Amgen Inc, said in January that it expects biosimilar launches in multiple countries in 2018.

“Unfounded fears on U.S. Humira growth were put to bed,” Jefferies analyst Jeffrey Holford said in a client note titled “Humira bears left out in the cold”.

The suburban Chicago drugmaker’s cancer drug Imbruvica raked in first-quarter sales of $ 551 million, while its hepatitis C treatment, Viekira Pak, brought in $ 263 million.

Net profit rose to $ 1.71 billion, or $ 1.06 per share, in the first quarter ended March 31, from $ 1.35 billion, or 83 cents per share, a year earlier.

Excluding items, the company earned $ 1.28, beating the average analyst estimate by 2 cents, according to Thomson Reuters I/B/E/S.

The profit beat was driven by a lower tax rate despite weaker-than-expected gross margins, Jefferies’ Holford said.

Net revenue rose 9.7 percent to $ 6.54 billion, coming in just ahead of the average estimate of $ 6.48 billion.

Shares of the company, which maintained its 2017 adjusted profit forecast, were up 1.4 percent at $ 66 in light premarket trading on Thursday.

The world’s largest drugmaker just gave us a better idea of how drug price hikes impact its business. 

Johnson & Johnson published a report on Monday detailing its average list price compared to its average net price after rebates and discounts. The companies’ drugs had an average list price increase of 8.5% in 2016 while its net price increase was just 3.5%. To recap, the list price is the amount a drugmaker sets, and it’s often the most publicly available number, while a net price is the amount it actually receives in return for the drug after factoring in any rebates or discounts. J&J said the report is now going to be annual.

Since 2012, the average yearly list price (that’s accounting for all of the products in J&J’s Janssen pharmaceutical unit) has increased between 7.6% to 9.7%, while the net price after rebates and discounts ended up increasing between 2.5% and 5.2%. 

“Every part of the health care system, including pharmaceutical companies like Janssen, should be held accountable for the value it delivers,” Janssen executives wrote in a letter.

In January, Merck published a similar report that outlined the company’s average list and net price increases for its products. Merck’s average net prices increased 5.5% in 2016. 

The reports are part of an effort to diffuse criticism faced by the entire industry that drug prices are out of control. While it has been a hotly covered topic for several years, President Donald Trump has been raising the stakes, saying that drug companies are “getting away with murder,” and that the pricing has been “astronomical.”

Pharmaceutical companies have been criticized for their routine list price increases. But, as those companies have argued, the list price doesn’t tell the whole story. There are other players in the system that each take a piece, which means that what a drugmaker actually receives could be lower even as the list price rises.

But there’s a lack of transparency about the portions each player gets. That’s why reports like Merck’s and J&J’s are based on the average list and net price, rather than individual drugs. It’s something lawmakers have started going after, especially as the number of people on high deductible plans facing near-list prices for drugs continues to rise.
